To schedule an appointment for a basic treatment, please fill out the form below. You will be guided to the appointment booking once the form is submitted. If you do not see the time you prefer, you can still send in a request and our staff will try to accommodate your desired time to the best of their ability.

The amount for the requested service will be put on hold on your card, but the charge will not be completed until your appointment is confirmed by our staff. Your credit card information is transmitted via encrypted network of Authorize.net. We do not store your credit card information. After your credit card information is submitted, you will be directed to our online booking page.
